Community Health Partners for Sustainability works to increase access to primary and preventive health care for residents of public housing who struggle to maintain healthy lives. We accomplish this through strategic partnerships and peer-to-peer sharing of approaches that work. Community Health Partners for Sustainability fosters partnerships between housing authorities, health centers, and local community health leaders to ensure that residents of public housing access care that meets their needs.

National Conference - Scalable Innovations for Health Centers and Public Housing

Thursday and Friday, November 29th and 30th, 2012

This fall, Community Health Partners for Sustainability invites health center leaders, public housing organizations, and community and national decision-makers to Philadelphia, PA for our national conference, "Scalable Innovations for Health Centers and Public Housing."  The conference will highlight innovations with the promise to improve safety-net health care nationwide and will highlight leaders from across the country.  Please join us!

Webinar: Making Programs Stronger & More Sustainable: The why, what and how for implementing a dual emphasis program

Recent reductions in state and federal funding for chronic disease prevention have increased the need for partnerships to make achievement of program objectives possible.  The Health Promotion Council developed a programmatic integration strategy targeting community based organizations working in tobacco control and chronic disease using a collaborative approach to capacity building. Through its statewide initiatives “BE-A-BRIDGE” and “PA cAARds!”, HPC has worked organizations to better integrate chronic disease and tobacco control programs. These initiatives created a vehicle for interdisciplinary communication, strategy development and collective solution building by hosting regional summits and workshops across the commonwealth and in doing so, created statewide infrastructure opening lines of communication between local and state government for the development of tools and resources.  To register for the webinar, click here.
